Website & case submission privacy notice

CLIK Dental Lab uses the information submitted through this website to review, communicate about and complete laboratory cases requested by dental practices.

Information you submit

The case form may include practice contact details, a case ID or patient initials, prescription notes and digital files supplied by the submitting dental professional.

Minimize patient identifiers

Please submit only the information reasonably necessary for laboratory work. Use a case ID or initials where practical and avoid unnecessary identifiers such as a full date of birth, health card number or other unrelated personal information.

Transmission and retention

Website case submissions are processed and stored through Netlify Forms. Uploaded files are available through the Netlify submission record, and the lab may receive an email notification containing the submitted information and links to uploaded files. CLIK Dental Lab may also retain case information in its ordinary laboratory and communication systems as needed for the case.

Sensitive information and file uploads

Do not submit unnecessary direct patient identifiers. Before uploading, remove full patient names, full dates of birth, health-card numbers and unrelated identifiers from filenames, notes and exported files where practical. For large or especially sensitive digital records, use the scanner portal or another workflow agreed with the laboratory.

Dental practices remain responsible for any authorization, consent and privacy obligations that apply to the information they submit.
